Covers oncology imaging (primarily conventional imaging such as CT, sometimes MRI or bone scan, and advanced imaging where indicated) for initial staging, re‑staging, and surveillance across a comprehensive set of cancer‑specific guidelines (e.g., CNS, lung, breast, colorectal, sarcoma, thyroid, etc.). Major limitations/requirements: age at initial diagnosis determines whether adult or pediatric guideline sets apply, a recent clinical evaluation (within 60 days) is required before advanced imaging, physician review is needed for atypical presentations, advanced imaging for staging is generally approved once when standard therapy is being considered, and imaging is not indicated for patients who decline standard oncologic therapy or are in palliative/hospice care; specific guideline sections may override these rules.
