Cigna covers manipulation of the spine under anesthesia (MUA) only as an emergent closed treatment of traumatically induced vertebral fracture or dislocation to mitigate potential neurological compromise when open reduction has been considered, and excludes MUA for non‑traumatic indications or when performed in isolation without concurrent rehabilitation. Key requirements: performed with an active rehabilitation/home exercise program and documented with emergency/urgent notes and imaging confirming fracture/dislocation, documentation that open reduction was considered, an anesthesia record, PT/home program evidence, and an operative/procedure note (CPT 22505).
