CMM-202: Trigger Point Injections

Trigger point injections are medically necessary when a myofascial trigger point is identified on exam (one or more of referred pain, tenderness, muscle shortening, or autonomic phenomena) and the injection uses a local anesthetic with or without corticosteroid. Repeat injections require documentation of ≥50% pain relief with functional improvement for ≥6 weeks and adequate self‑management instruction, must be ≥2 months apart and ≤4 sessions per body region per year, and injections using other substances (e.g., saline/glucose), or performed as acupuncture/electro‑acupuncture/acupoint/biopuncture/dry needling, image‑guided over spinal hardware, same‑day other regional treatments, or as isolated treatment without rehab are excluded.
"Repeat trigger point injections are considered not medically necessary if the interval between injections is less than two (2) months."
