Cardiac Rhythm Implantable Devices (CRID) Guidelines

This policy covers indications for cardiac implantable devices—including ICDs (definite and reasonable indications), CRT‑D/CRT‑P, permanent and leadless pacemakers—for patients with structural heart disease, heart failure, ventricular arrhythmias (NSVT, incessant VT), long QT syndrome, abnormal exercise blood pressure responses, and related symptom complexes. Major requirements/limitations include documentation of three months of optimally titrated medical therapy for heart failure before device consideration, physician review for atypical presentations, and that payer/CMS coverage policies and clinician judgment may supersede or modify these guidelines.
