High-frequency chest wall oscillation systems are covered only for patients with documented neuromuscular disease or cystic fibrosis plus imaging-confirmed bronchiectasis and one of: daily productive cough, frequent antibiotic-treated exacerbations, or documented failure/intolerance/contraindication to standard airway clearance (e.g., chest percussion/postural drainage); absence of these criteria (including no bronchiectasis) is non-covered. Coverage requires documentation of the diagnosis, radiology report of bronchiectasis, clinical notes/prescriptions showing symptoms or prior treatment failure/intolerance/contraindication, and billing is limited to HCPCS E0483, A7025, A7026.
