Electrical stimulation is covered as an adjunctive home-based therapy only for chronic stage 3–4 pressure ulcers, arterial, diabetic, and venous stasis ulcers after a comprehensive wound therapy program has been attempted without measurable improvement (not covered for stage 1–2 pressure injuries or when the wound is 100% epithelialized or continued-rental criteria are not met). Key requirements: initial 30‑day rental, supervision by a trained medical professional, documentation that prior therapies (dressings, debridement, infection/underlying condition management, offloading/compression/circulation/diabetes care as applicable) were tried, and monthly wound measurements/progress documentation for continued rental with additional detailed monthly justifications required beyond four months.
