Key requirements: initial 30‑day rental, supervision by a trained medical professional, documentation that prior therapies (dressings, debridement, infection/underlying condition management, offloading/compression/circulation/diabetes care as applicable) were tried, and monthly wound measurements/progress documentation for continued rental with additional detailed monthly justifications required beyond four months.
