Replacement and Continued Use of DME - DME.AD.101.A

Continued rental beyond the initial period and replacement of DME are covered only with required documentation and otherwise excluded for routine replacement or for devices still under warranty or that are repairable. Continued rental requires treating-practitioner re-evaluation and documentation of continued use and medical need; replacement requires practitioner evaluation, documentation of continued need, plus documentation of loss/theft, significant clinical change rendering the device ineffective, or supplier-confirmed unrepairable device with no remaining warranty.
