Massage therapy is covered only when medically necessary for primary neuromusculoskeletal injuries/illnesses (e.g., neck, back, shoulder/limb pain, fibromyalgia) for up to 60 minutes per session by participating therapists, while preventive/maintenance care, services related to auto or workers' compensation, non‑participating providers, experimental/investigational services, inpatient care, supplies (e.g., lotions), TENS/electrical devices and many other specified services are excluded. Coverage requires documented subjective complaints and baseline functional limitations with objective progress notes demonstrating significant, lasting therapeutic benefit and improvement (with expected progress milestones), and prior authorization is required for continued services (initial visit or emergencies excepted).
