Monoferric® (ferric derisomaltose)

Monoferric is covered for adults (≥18 years) for FDA‑approved uses (iron deficiency anemia with intolerance/inadequate response to oral iron; non‑hemodialysis CKD) and select off‑label compendial uses (CKD on dialysis; iron deficiency in heart failure); pediatric patients (<18) are not covered. Key requirements: specialty prescriber involvement for CKD (nephrologist/hematologist) or heart failure (cardiologist/hematologist), documentation of diagnosis and prior oral‑iron trial or qualifying exception (intolerance, malabsorption, ESA use, or cancer/chemotherapy), weight‑based single IV dosing (≥50 kg = 1,000 mg; <50 kg = 20 mg/kg) infused ≥20 minutes, repeat dosing only if anemia recurs, and approvals limited to 12 months.
