Monoferric® (ferric derisomaltose)

Monoferric IV is covered for adults (≥18) with iron deficiency anemia — FDA‑approved for those intolerant of or refractory to oral iron and for non‑hemodialysis CKD, with compendial coverage for dialysis‑dependent CKD, heart‑failure–related iron deficiency and other adult indications (e.g., failed oral iron, malabsorption, on ESAs, or cancer/chemotherapy‑related anemia); it is not covered for patients <18 years. Coverage requires documented diagnosis and prior oral‑iron trial or qualifying exception, specialist prescribing/consultation for CKD (nephrologist/hematologist) and HF (cardiologist/hematologist) when applicable, weight‑based single IV dosing (1,000 mg if ≥50 kg; 20 mg/kg if <50 kg) infused ≥20 minutes, documentation of weight and infusion, approval up to 12 months, and repeat dosing only if anemia recurs.
