AbobotulinumtoxinA (Dysport)

Dysport is covered only for cervical dystonia, adult upper/lower limb spasticity, pediatric lower‑limb spasticity, blepharospasm, and hemifacial spasm (other uses are not covered), with specific maximum doses, per‑site volume limits, routes (IM for dystonia/spasticity; SC for blepharospasm/hemifacial spasm) and frequency limits (generally ≥12 weeks between injections; hemifacial spasm ≥3 months). Approval (up to 12 months) requires documentation of diagnosis, dosing (including pediatric weight‑based calculations and dose caps), injection dates/sites/volumes, prior injection intervals, and that applicable safety criteria are met.
