Policy Summary
Manipulation under anesthesia (CPT 22505) is covered only as a closed, emergent treatment to mitigate neurological compromise from traumatically induced vertebral fracture or dislocation and is not medically necessary for non‑traumatic or non‑emergent spinal conditions or when performed in isolation. Coverage requires documentation that the injury is traumatic and emergent, that a qualified physician considered open reduction, and that the procedure will be performed with an active rehabilitation/therapeutic exercise program including a home exercise component.
