CMM-406: Arthroscopy Ankle

Ankle arthroscopy procedures (including arthroscopically aided repair CPT 29892, endoscopic plantar fasciotomy CPT 29893, and other ankle arthroscopies) are covered only for specified diagnoses (e.g., OCD, talar/tibial plafond fractures, recalcitrant plantar fasciitis, mechanical ankle pathology) with documented qualifying subjective symptoms, objective exam findings, and imaging confirmation when required. Key requirements: documented failure of conservative therapy (generally ≥6 weeks with specific bracing/walker-boot and steroid-injection stipulations), preoperative imaging, documentation of <50% improvement, and exclusion when contraindications or red-flag/urgent conditions exist (e.g., active infection/septic arthritis, peripheral vascular disease, uncontrolled comorbidities, fractures with loose fragments, locked joint, or inability to comply with postoperative restrictions).
