Covered only when manipulation under sedation or general anesthesia is used as a closed treatment for traumatically induced vertebral fracture or dislocation in an emergent situation to mitigate neurological compromise after a qualified physician has considered open reduction; it must be performed with an active rehabilitation/therapeutic exercise and home program and fully documented (traumatic injury, emergent status, consideration of open reduction, closed procedure, and anesthesia records). Excluded/not medically necessary for non‑traumatic or routine spinal conditions or when performed in isolation (without concurrent rehab/home exercise).
