Non‑high‑frequency dorsal column SCS (with HF10 only in a limited role) is medically necessary for short‑term trial (>48 hours) and permanent implantation for FBSS with intractable neuropathic leg pain, CRPS of the extremities, chronic critical limb ischemia, and chronic stable angina when indication‑specific objective criteria are met — typically requiring ≥6 months of failed physician‑supervised conservative therapy where applicable, a behavioral‑health attestation of no uncontrolled psychiatric issues, and demonstration of ≥50% pain reduction during the trial prior to permanent implantation. Most other technologies/uses (including DRG stimulation for most indications, high‑frequency SCS outside its limited role, burst/other generator modes, peripheral nerve stimulation, post‑amputation pain, neuropathies, and replacing a functioning non‑HF SCS with HF SCS) are considered experimental/investigational/unproven or not medically necessary, and all implants require supporting documentation.
