CMM-202: Trigger Point Injections

Trigger point injections with local anesthetic ± corticosteroid are medically reasonable when a discrete myofascial trigger point is documented by focal tenderness with characteristic referred pain and preferably a local twitch response, while injections using other agents (e.g., saline, glucose) or repeat injections as an isolated treatment are not recommended/considered not medically necessary. Repeat injections are only covered if the prior injection produced ≥50% pain relief for ≥6 weeks with documented functional improvement and self‑management instruction, performed ≥2 months apart, ≤4 sessions per body region per year, and all findings, timing, and agents must be documented.
