Manipulation of the spine under sedation/general anesthesia is covered only as a closed treatment for emergent, traumatically induced vertebral fracture or dislocation to prevent neurological compromise and is not covered for non‑traumatic or routine spinal conditions. Key requirements: open reduction must have been considered by a qualified physician, the procedure must be performed with an active rehabilitation/therapeutic exercise (and home exercise) program, and documentation must confirm the trauma, emergent status, anesthesia/procedure details, physician decision‑making, and rehab participation.
