CMM-202: Trigger Point Injections

Covered: trigger point injections for clinically identified myofascial trigger points using a local anesthetic with or without corticosteroid; excluded are other injectates/techniques (dry needling, acupuncture/acupoint injections/Biopuncture, electro‑acupuncture), image‑guided injections over hardware, and more than one invasive procedure on the same date of service. Key requirements: documentation of one or more exam findings (referred pain, tenderness, muscle shortening, or autonomic phenomena) and use of local anesthetic ± steroid; repeat injections require ≥50% pain relief with functional improvement for ≥6 weeks, participation in a conservative adjuvant‑care program, ≥2 months between sessions, and a limit of four sessions per body region per year.
