Covers short-term (≈3–7 day) trials and permanent dorsal column SCS implants for Failed Back Syndrome with neuropathic leg pain, CRPS/RSD, chronic critical limb ischemia, and chronic stable angina (CCS III–IV), plus replacement of malfunctioning devices not under warranty; peripheral nerve stimulation, high-frequency SCS, and dorsal-column SCS for other indications (e.g., post‑amputation pain, post‑herpetic neuralgia, peripheral neuropathy, spinal cord injury dysesthesias) are investigational/unproven. Key requirements: documented failure of conservative therapy (≥6 months) and relevant surgical/revascularization attempts when applicable, a mental health evaluation without contraindicating conditions, documentation of trial duration/response (≥50% pain reduction for FBS/CRPS or other documented beneficial response for CLI/angina) prior to permanent implantation, and proof replacements are out of warranty.
