Addendum to eviCore Guidelines

This addendum (effective 1/1/2020) updates eviCore coding—adding new CPTs (notably SPECT/CT 78830–78832 and revised 78800–78804/78803, cardiac PET 78429–78434, duplex pre‑op mapping 93985/93986, SI‑joint injections/RFA 64451/64625, myocardial strain, etc.) and retiring multiple legacy CPT/HCPCS codes (e.g., 78320, 78805–78807, 0399T); deleted codes must not be used after 1/1/2020 and replacement codes listed in the addendum must be reported. Clinical coverage, medical‑necessity determinations and documentation requirements are unchanged by this addendum and remain governed by the referenced underlying eviCore guideline sections.
