Addendum to the eviCore Musculoskeletal Advanced Procedures Guidelines

Effective 1/1/2021 eviCore removed transforaminal epidural ultrasound‑guided CPT codes 0228T–0231T from the "Experimental, Investigational or Unproven" list in CMM‑200 (so they are no longer considered experimental/unproven under that guideline and not excluded on that basis), and added intradiscal allogeneic injection codes 0627T–0630T to the "Experimental, Investigational or Unproven" list in CMM‑308 (so those procedures are now considered experimental/investigational/unproven and excluded under that guideline). These changes apply specifically to the CMM‑200 and CMM‑308 guidelines and are effective January 1, 2021.
