Manipulation under anesthesia (CPT 22505) is covered only as a closed, emergent treatment for traumatically induced vertebral fracture or dislocation to mitigate neurological compromise and is not medically necessary for non‑traumatic, non‑fracture/dislocation, non‑emergent, chronic indications or when performed in isolation. Key requirements: a qualified physician must have considered open reduction, the patient must be sedated or under general anesthesia, and documentation must confirm emergent status plus that the procedure is performed with an active rehabilitation/therapeutic and home exercise program.
