CMM-202: Trigger Point Injections

Trigger point injections are medically necessary only when a clinically identified myofascial trigger point is documented (referred pain, tenderness, muscle shortening, or autonomic phenomena) and the injection is performed with a local anesthetic with or without corticosteroid. Repeat injections require ≥50% pain relief with documented functional improvement for ≥6 weeks plus instruction in self‑management, and injections are not covered if done with non‑anesthetic substances (e.g., saline/glucose), on the same day as other treatments in the same region, for acupuncture/biopuncture/dry needling, image‑guided over spinal hardware, with <2‑month intervals, or >4 sessions per body region per year.
