Clinical Information to Establish Medical Necessity

eviCore evaluates medical necessity for requested tests and treatments only with submission of legible, detailed medical records—such as a recent history & physical, laboratory studies, imaging, pathology and procedure reports, and other providers’ notes—and will not approve requests when required clinical information is missing or illegible.
