Pediatric and Special Populations Oncology Imaging Guidelines

This policy covers medical imaging (conventional and advanced modalities such as CT, MRI, PET) for pediatric and special-population oncology indications, with disease-specific guidance for pediatric cancers (e.g., leukemias, CNS tumors, lymphomas, neuroblastoma, renal and soft‑tissue sarcomas, retinoblastoma, and many others). Major limitations/requirements include that atypical or unaddressed presentations require physician review; a relevant clinical evaluation and usually histologic confirmation and staging must be provided prior to advanced imaging; and advanced or routine surveillance imaging of asymptomatic individuals and repeat studies are generally not medically necessary unless specified by disease-specific guidelines or when results will affect management.
