Pediatric and Special Populations Oncology Imaging Guidelines

This policy covers oncology imaging guidance for pediatric and special populations, addressing appropriate use of conventional and advanced imaging across childhood cancers (e.g., leukemias, CNS tumors, lymphomas, neuroblastoma and other pediatric solid tumors). Major limitations/requirements include a recent clinical evaluation or meaningful contact within 60 days before advanced imaging, histologic confirmation and staging (unless a disease‑specific exception applies), routine or repeat imaging and asymptomatic surveillance generally not supported, and atypical presentations require physician review; PET is limited for ongoing surveillance once PET‑negative or PET‑avid disease has been resected.
