Pediatric Peripheral Vascular Disease (PVD) Imaging Guidelines

This policy covers pediatric peripheral vascular disease imaging — including MRA, CTA, PET/CT (select codes), duplex and noninvasive physiologic studies — for evaluation of vascular anomalies, vasculitis, disorders of the aorta and visceral/limb arteries and other peripheral vascular conditions in patients under 18. Imaging is limited to patients with documented active signs or symptoms and generally requires a recent (≤60 days) face‑to‑face evaluation; repeat studies need evidence of progression or impact on management, age‑based routing mandates pediatric guidelines for <18, certain PET codes are not used or are rarely used in pediatrics, and contrast contraindications and anesthesia (usually required for most children <7) must be considered with efforts to minimize exposure.
