Covers pediatric musculoskeletal imaging services (plain radiography, ultrasound, CT, MRI, nuclear medicine/SPECT/PET and associated CPT codes) for common pediatric MSK presentations including fractures/dislocations, soft tissue and bone masses, limp, developmental dysplasia of the hip, AVN/Legg‑Calvé‑Perthes, suspected child abuse, infection/osteomyelitis, SCFE, inflammatory and tendon injuries, and other pediatric indications. Major requirements/limitations: basic imaging (plain x‑ray or ultrasound) must generally precede advanced imaging (unless guideline‑scheduled), prior x‑rays must be available, atypical presentations require physician review, contrast MRI and many PET uses are rarely/not used in pediatrics, SPECT has area/CT requirements, and repeat imaging is limited unless there is documented progression or impact on management.
