Peripheral Nerve and Neuromuscular Disorders (PNND) Imaging Guidelines

This policy covers diagnostic imaging for peripheral nerve and neuromuscular disorders, including focal neuropathy, polyneuropathy, brachial and lumbosacral plexus conditions, muscle disorders, peripheral nerve sheath tumors, and magnetic resonance neurography. Advanced imaging requires a prior pertinent clinical evaluation (history and neurologic exam, appropriate labs, non‑advanced imaging, and EMG/NCV or acceptable telehealth/other technological contact); ultrasound is preferred for superficial nerves, MRI is used for deep or bone‑obscured nerves or surgical planning, CT is limited to MRI contraindication, and atypical presentations need physician review.
