Preface to the Imaging Guidelines

This preface covers EviCore’s evidence‑based Clinical Review Criteria for advanced imaging and related procedures — including radiography, ultrasound, CT, MRI, PET, nuclear medicine, radiation oncology, sleep studies, GI endoscopy, and cardiac, musculoskeletal and spine interventions — applied to common symptom complexes and specific populations (pediatric patients per Pediatric Imaging Guidelines, adults >18, and clinical trial imaging that meets health plan coverage). Use is limited to individual clinician determinations for delegated patients; approvals require adequate clinical information and a pertinent clinical evaluation when specified (face‑to‑face or other meaningful contact), atypical presentations require physician review, and payor benefit/coverage rules and CRC proprietary restrictions may limit applicability.
