Preface to the Imaging Guidelines

This policy provides EviCore’s evidence‑based imaging and procedure guidelines and Clinical Decision Support for a broad array of diagnostic and interventional services—including nuclear medicine, ultrasound, CT, MRI, PET, radiation oncology, sleep studies, GI endoscopy, cardiac, musculoskeletal and spine interventions—for patients presenting with common symptoms and symptom complexes. Major limitations/requirements include the need for adequate clinical information and that studies be used in direct clinical management (with x‑ray prerequisites for spine/MSK), exclusion of investigational/experimental or unsupported clinical‑trial imaging, adherence to payer/Medicare/Medicaid rules, and allowance for physician judgment or review in atypical cases.
