Preface to the Imaging Guidelines

This policy covers evidence‑based prefatory imaging guidelines for a broad range of diagnostic and advanced imaging services — including ultrasound, nuclear medicine, CT, MRI, PET, radiation oncology, sleep studies, and cardiac and spine interventions — to help select appropriate imaging for common symptoms and symptom complexes. It applies to the majority of patients (including consideration of CMS/Coverage‑with‑Evidence‑Development trials for Medicare enrollees, where CMS policies prevail), but atypical presentations require physician review, payer‑specific or investigational exclusions may limit coverage, and imaging should follow clinical evaluation with clinician judgment able to override the tool.
