This policy covers imaging and noninvasive physiologic testing for peripheral vascular disease (PVD), including arterial and venous studies such as duplex ultrasound, MR venography, ankle‑brachial and toe‑brachial indices (for non‑compressible arteries), V/Q scanning, and CPT‑coded physiologic studies (e.g., CPT 93922, 93923, 93924). It applies to symptomatic patients with suspected PAD/PVD and related conditions but includes CPT‑specific limits and requirements (single‑level vs complete bilateral studies, prohibitions on billing certain codes together), notes that ABI testing with handheld Dopplers lacking hard‑copy flow output is not reportable, and requires physician review for atypical presentations.
