Peripheral Vascular Disease (PVD) Imaging Guidelines

This policy covers imaging and noninvasive vascular testing for peripheral vascular disease—including ankle‑brachial index testing, arterial and venous Doppler and advanced imaging, nuclear medicine studies, and evaluations related to carotid/cerebrovascular disease, aortic/renal/visceral aneurysms, pulmonary hypertension, AVMs, hemodialysis access, IVC filters and post‑procedural surveillance. It applies to patients with suspected PAD/aneurysm, claudication, critical limb ischemia, DVT or chronic venous insufficiency (including venous stasis ulcers and May‑Thurner) and post‑stent/angioplasty follow‑up, but advanced imaging requires a current clinical evaluation within 60 days documenting medical treatments (exercise ABI only if resting ABI is normal), atypical presentations need physician review, and specific billing limits (e.g., CPT® 93922/93923) apply.
