Air Ambulance Transportation (AAT) - MEDICAID - LOUISIANA

This policy covers air ambulance transportation (rotor- and fixed‑wing) for Humana Healthy Horizons Medicaid members in Louisiana, permitting emergency air transports without prior authorization and medically necessary non‑emergent transports when prior authorization is obtained. Coverage is limited to situations where land transport is infeasible or timely admission is essential, is contingent on medical necessity and appropriate BHSF/LDH certification/licensure of aircraft and personnel, excludes outpatient air transports, and requires separate reimbursement/authorization handling when both ground and air transports occur.
