This Humana Medicare Advantage coverage article states that ambulatory cardiac monitoring devices (Holter, patch, external loop/event monitors, MCOT, and implantable loop recorders) are used to detect and manage cardiac arrhythmias and related conditions (eg, cryptogenic stroke, palpitations, syncope) and that medical necessity is determined per applicable CMS NCDs/LCDs (notably LCD L34636 for external monitors and L39490 for implantable monitors). Removal/replacement of ILRs is reasonable when monitoring duration is complete or for battery end/device malfunction if the ILR remains medically necessary and duration aligns with signs/symptoms; ILR replacement is not reasonable to evaluate AF burden when an implanted device with an atrial lead can provide that information.
