This policy covers Medicaid-funded Applied Behavioral Analysis (ABA) services for Humana Healthy Horizons® members in Louisiana, including design, implementation, and evaluation of ABA-based therapies (with functional assessment and direct observation) to address socially significant behavior deficits or excesses, commonly for conditions such as autism spectrum disorder. Coverage is limited to medically necessary, non‑experimental ABA delivered per a prior‑authorized behavior treatment plan for members under 21, requiring a completed comprehensive diagnostic evaluation (CDE) by a qualified health care professional and provision/supervision and billing by a licensed behavior analyst, licensed psychologist, or licensed medical psychologist.
