This policy covers carotid revascularization procedures—including carotid endarterectomy (CEA), carotid angioplasty and stenting (CAS/PTA) and related stent placement—for management of extracranial carotid artery atherosclerotic stenosis in symptomatic patients (eg, TIA, amaurosis fugax, ischemic stroke within 6 months) and select asymptomatic patients. Coverage is governed by CMS NCD 20.7 and guideline-based criteria (eg, CEA generally indicated for symptomatic ≥50% stenosis and asymptomatic patients with higher-grade stenosis after optimal medical therapy); CEA is not recommended for symptomatic patients with <50% stenosis or those with recent severe stroke-related disability (Modified Rankin Scale ≥3), evidence is limited for chronic total occlusion and high‑risk scenarios, and listed CPT/HCPCS codes are informational only.
