Chest Computed Tomography (CT) - Medicare Advantage

This policy covers chest computed tomography (CT), including low‑dose CT (LDCT), for Medicare Advantage members for lung cancer screening and diagnostic evaluation—notably Lung‑RADS findings, surveillance and follow‑up of incidentally detected pulmonary nodules, and certain preoperative/preprocedural thoracic indications. Coverage is limited to indications that meet Medicare NCD/LCD/LCA criteria (or Humana’s interpretation in jurisdictions without an LCD), requires demonstration of medical necessity per applicable MAC LCDs, and services not meeting those criteria are not covered.
