Code Compendium (Musculoskeletal and Neurologic) - Medicare Advantage

This Humana Medicare Advantage policy (effective 12/01/2025) states there are no covered indications for autologous stem cell therapies, hydrodissection, the medial knee extraarticular implantable shock absorber (Misha knee system), or robotic assisted in-home stroke therapy systems. These services are considered not medically reasonable and necessary for musculoskeletal and neurologic indications due to insufficient evidence; specific CPT/HCPCS codes referenced in the exclusions include 38232, 0565T, 0566T, 0717T, 0718T, C8003, E0738, and E0739.
