This Humana Medicare Advantage policy covers comprehensive genomic profiling (CGP) and multigene panel testing of somatic tumor DNA/RNA for solid tumors to detect actionable alterations (e.g., NTRK/TRK fusions) to guide targeted therapy. It applies to Medicare Advantage members with advanced, unresectable, or metastatic solid tumors across many tumor types, but coverage is limited to somatic testing for solid tumors (not germline or hematologic indications) and is subject to the applicable MAC LCD/LCA, CMS National Coverage Determinations, and jurisdictional contractor requirements.
