This policy covers cosmetic and reconstructive surgery including scar revision, reconstruction for bodily injury, congenital anomalies, infection or other disease, removal of cutaneous vascular lesions (eg, infantile hemangioma, port‑wine stain), rhinophyma excision, and limited chemical peel/dermabrasion for actinic keratoses. Coverage is limited to medically necessary reconstructive procedures with an objective, measurable functional impairment (scar revision generally only if from accidental injury or prior medically necessary surgery); vascular lesion treatment requires failed/contraindicated conservative therapy or high‑risk features (vital‑structure involvement, ulceration, recurrent bleeding), and purely cosmetic procedures are excluded.
