Doula Services as an In Lieu of Service - MEDICAID - LOUISIANA
HUMANA-DOULA-SERVICES-AS-AN-IN-LIEU-OF-SERVICE-LA-MEDICAID
This policy covers doula services (prenatal visits, delivery attendance, and postnatal advocacy visits) provided as an in-lieu-of service for Medicaid members enrolled in Humana Healthy Horizons in Louisiana. Coverage is for expectant and postpartum parents with limits—typically up to five prenatal S9445 visits, up to three postnatal S9445 visits, and one doula delivery attendance (CPT 99199)—and requires doulas to be non‑clinical, in good standing on the Louisiana DOH doula registry, enrolled with Louisiana Medicaid, billed per HHH requirements, and provided as a supplement (not a replacement) to routine obstetrical care.
"Doula providers may not have been excluded, barred, suspended, or otherwise lawfully prohibited from participation in any government healthcare program."
