Foot Surgical Procedures - Medicare Advantage

This Medicare Advantage policy covers surgical foot procedures and related implants—such as tendon/ligament shortening or lengthening, amputations, osteotomies, arthrodesis/arthroplasty, and metatarsal/hallux/prosthetic implants (including unlisted foot/toe procedures). Coverage applies to symptomatic or functionally limiting conditions (e.g., bunion/hallux valgus, MTP subluxation/dislocation, hammer toe, hallux limitus/rigidus, ulceration) when medically necessary, excludes asymptomatic or purely cosmetic corrections, and is subject to member plan rules, medical necessity review, and applicable CMS/NCD/LCD guidance (prior conservative treatment is generally expected).
