Functional Family Therapy - MEDICAID - LOUISIANA

This policy covers Functional Family Therapy (FFT) and Functional Family Therapy–Child Welfare (FFT‑CW), an intensive in‑home, family‑based treatment for youth exhibiting significant externalizing behaviors (FFT for ages 10–18; FFT‑CW for ages 0–18) and their families, including child‑welfare cases. Coverage requires documented medical necessity, at least one adult caregiver willing/able to participate, that impairment is not solely due to autism spectrum disorder or intellectual disability, and services are limited when another level of care is indicated, program components are already completed, plan eligibility conditions apply (Humana Healthy Horizons LA), and FFT may not be billed with PRTF services.
