Covers genetic testing to detect DNA variants for diagnosis and treatment decision-making of inherited conditions, including mitochondrial disorders, hematologic disorders (neutropenia, thrombocytopenia, thrombophilia, hemophilia), hematologic malignancies, CANVAS, DRPLA, glycogen storage disease type I, intellectual disability, corneal dystrophy and many other suspected hereditary disorders. Major limitations: testing is intended for individuals who exhibit clinical signs and symptoms (not routine asymptomatic screening), is not indicated for acquired (non‑inherited) coagulation disorders, and specific exclusions, frequency limits and other requirements are detailed in the policy’s Coverage Limitations section and may be modified by the member’s benefit document.
