Genetic Testing for Hereditary Cancer - Medicare Advantage

This policy covers molecular and laboratory‑developed genetic testing (MDTs/LDTs and FDA‑cleared/approved tests) for hereditary cancer syndromes, including multi‑gene panels (e.g., hereditary pan‑cancer and neuroendocrine tumor panels) performed on blood or saliva. Coverage applies to Medicare beneficiaries with suspected or diagnosed cancer when testing is reasonable and necessary — tests must demonstrate analytical and clinical validity and clinical utility per the MolDX technical assessment (FDA‑cleared tests may be exempt if used strictly within labeling) and are subject to MolDX/Noridian jurisdictional rules and referenced LCDs for specific limitations.
