This Medicare Advantage policy covers genetic and molecular testing (targeted single‑gene tests and multigene panels) for hereditary cardiovascular diseases, including hypertrophic cardiomyopathy, inherited cardiomyopathies, inherited aortopathies (excluding Marfan syndrome when clinical criteria are met), inherited lipidemias, and inherited cardiac channelopathies. Coverage is limited to individuals with a relevant personal or family history or clinical indication, is governed by MolDX/MAC LCD guidance and jurisdictional limits, reserves expanded panels for cases with multiple suspected genes/conditions, and excludes testing for acquired (non‑inherited) cardiac disease; FBN1 testing is not indicated when clinical diagnostic criteria for Marfan syndrome are already met.
