Covers surgical treatments for glaucoma — including micro‑invasive glaucoma surgery (MIGS) and combination cataract/MIGS procedures — as specified in the referenced CMS Local Coverage Determinations (LCDs). Applies to patients with glaucoma (e.g., open‑angle, angle‑closure/narrow‑angle, primary open‑angle, refractory, and varying disease stages) for whom surgery is clinically indicated to prevent vision loss. Coverage, coding (including Category III code considerations), and any exclusions or device/IDE restrictions are determined by the applicable MAC LCD in the patient’s jurisdiction and typically require documentation of prior adequate medical and/or laser therapy and, where specified, prior failure of filtering or cilioablative procedures.
