Humana’s Medicare Advantage policy covers surgical, injection, neurolytic and neuromodulation treatments for headache and occipital neuralgia — examples include occipital nerve blocks and denervation, peripheral/implantable occipital nerve stimulation, surgical decompression/transposition/vascular ligation, and botulinum toxin per LCD L34635. Coverage is limited to patients with chronic headache, occipital neuralgia, treatment‑resistant migraine, cervicogenic headache or postsurgical neuropathic pain and is governed by Medicare NCDs/LCDs and MAC jurisdictional criteria (occipital nerve blocks may be used to confirm diagnosis but not as definitive treatment, pulsed RFA is excluded, and all services require documented medical necessity per the referenced LCDs).
